Our Patient Portal is designed for our established patients. Through our patient portal you may securely access your personal health records.
You may be able to view and manage changes to your demographic information (address, phone numbers, etc.). You will also be able to view your appointments, clinical summaries**, statements, and other documents.
You don’t need to have an email address to be able to obtain your login information to access the patient portal, but you will need a username and password. New patients may receive their login information at the end of their initial visit with one of our providers.
